No. The game was suppressed during the Cultural Revolution and has been played openly since the early 1980s, in homes, tea houses and parlours across the country. What local authorities restrict from time to time is gambling attached to the game rather than the game itself. The rules played there are the Chinese ones this set is built for: 144 tiles, no jokers, flowers and seasons included.
It describes how numbered tiles group into families three apart: 1, 4 and 7, then 2, 5 and 8, then 3, 6 and 9. A two sided wait always sits on two numbers from the same family, so if a 4 has already been discarded and nobody claimed it, the 1 and the 7 of that suit are comparatively safe to let go. It applies only to the numbered suits, 108 of the 144 tiles here, never to winds or dragons.
It began as a 1920s craze and stayed, largely because the game suited a standing weekly gathering: four players, a fixed table, conversation between hands. In American Jewish communities that weekly game became a tradition carried down through families. The version played is the American one, with 166 tiles, jokers and an annual card of hands, so it needs different equipment from this 144 tile set.
Research on older adults links regular mahjong to modest gains in attention and short term memory, and studies usually credit the social side as much as the game itself. It is not a treatment and nobody should read it as one.
